Customer Experience (CX) Data

The State of Utah gives you the opportunity to share feedback about its services in short (even one-question) surveys as part of the Customer Experience initiative. These surveys are optional, and help enhance services. If you choose to respond, please don’t share sensitive information. 

The Customer Experience team partners with state agencies to collect feedback from the public about online and in-person interactions, empathy, satisfaction, reliability and effort scores.

How your feedback may be shared

De-identified feedback on state services may be used by relevant state agencies to identify ways to improve websites, call centers, online services, office visits and other state services. Data may also be shared to relevant state agencies to resolve service requests. It may be used to showcase the improvement to other state agencies, stakeholders, or the public.

How we handle personally identifiable information

In some situations, you may choose to provide your contact information to request help. Any personally identifiable information (PII) collected in this form or survey is handled per record series 30838. PII may include, but is not limited to, IP address, name, phone number, email address, and geolocation.

  • PII will be retained for one year or until the administrative need (like following up with a customer to resolve an issue) is met, whichever is less.
  • After the above timeframe, PII will be destroyed.
  • Retention and disposition for this series is authorized by Archives general schedule Transitory tracking records, GRS-1720.

How we handle other data

Any other information you share is considered raw information and is contained in large databases that grow incrementally and accumulate over time. The collected survey responses and resulting analyses and reports may be disclosed to the public in the form of periodic customized reports published by the agency(ies) involved.

Non-PII data is handled in accordance with record series 30837.

  • Feedback will be retained for 10 year(s) or until the administrative value is met, whichever is greater.
  • After the above timeframe, feedback data will be destroyed.
  • Retention and disposition for this series is authorized by Archives general schedule Program and project analysis records, GRS-1277.
