Report Accessibility Issues

This personal data request privacy notice is meant to help you understand what personal data we collect and why we collect it. The Report Accessibility Issues form requests personal data, IP address and browser user agent to facilitate communication with you regarding your message to the Division of Technology Services (DTS). Providing the requested personal data of your name and email address is optional and you do not need to provide it if you do not want a response to your message. If you would like a response from DTS, you need to provide at least your email address.

We collect your personal data for the purpose of communicating with you. We use your personal data to:

If you do not provide your contact information, DTS will not be able to respond to or communicate with you. Your personal data will be shared only where required by law.
