Report an Incident or Submit a Request

You now have more options when entering your own incident. You can report an incident (issue/problem) or submit a request. This method is most efficient for non-urgent requests/incidents (Remedy Tickets) and will require a UMD login.

If your issue is urgent, please contact the DTS Help Desk at 801-538-3440 or toll free at 1-800-678-3440.

Note: Incident Management has a NEW look! When you login, your User Name will be Upper and Lower case for example (John Doe). Then you will use your UMD password, this will take you to the new Service Request Management Screen.

You then select Get Help from the menu to get the following options:

  1. Report an Incident (issue/problem)
  2. Submit a Request (such as requesting a new service for a hardware install or move).
    NOTE: For Phone adds, moves, or changes, go to the Forms page and select "Telecommunications."
  3. Or choose one of the other specific services listed on the screen.
